Job Title: Patient Care Coordinator

We are seeking a skilled and compassionate Patient Care Coordinator to join our team. As a key member of our healthcare team, you will work collaboratively with patient care teams, community service providers, government agencies, and multi-disciplinary hospital and healthcare teams to provide coordination and continuity of patient care across the healthcare continuum.

  • Attend to medical queries received via a 24/7 telephone hotline, providing an avenue for caregivers and community partners to connect with us for information relating to patient health and wellbeing.
  • Evaluate medical queries, escalating them to appropriate medical providers and following up on case closure.
  • Perform triaging for transitional care referrals and direct patients to external providers when necessary.
  • Assess patient medical-nursing, psycho-social, functional status, and daily activity needs, as well as existing support system availability upon enrolment into program.
  • Provide guidance and assistance to colleagues in escalation of complex medical calls or referrals triaging when needed.
  • Implement care coordination and transitional case management, evaluating outcomes accordingly.
  • Synthesize assessment information to prioritize care needs and develop care plans and goals together with patients and/or families/caregivers, discussing with patient care teams and community partners involved.
  • Work in partnership with patients and families/caregivers to explore various services and options available in the patient's community, coordinating referrals and follow-ups in a timely manner.
  • Adopt a multi-disciplinary approach with focus on coordination support, making connections with transitional partners to facilitate support and assistance for individuals to address social and health issues.
  • Conduct follow-up phone calls and/or home visits to ensure smooth coping of patients and caregivers.
  • Promote positive changes in patient lifestyle in the community.
  • Monitor patient general medical condition during home visits and report to patient Principal Physician or primary care provider and/or community partner where necessary.
  • Educate and promote advanced care planning, assist patients and their families/caregivers in planning for and improving end-of-life care, ensuring choices are reflected in personalized care plans.
  • Document assessments, plans, and outcomes promptly and accurately in relevant systems.
  • Maintain high-level contact with step-down facilities.
  • Advocate for patients and their families/caregivers, forming strong relationships with community partners to work in patient best interests.
  • Participate in activities contributing towards improvement of patient care, including professional development sessions to develop relevant areas of knowledge, skills, and attitudes.
