Job Description

We are seeking a dedicated Care Support Professional to join our team. As a Care Support Professional, you will play a vital role in providing exceptional care and support to our patients and their families.

Key Responsibilities:

  • Assist in carrying out administrative tasks related to patient care and assistance.
  • Process information and statistics on assistance through an electronic system.
  • Gather information from patients and their families and assist them with financial assessments procedures.
  • Conduct assessment/home visits when necessary.
  • Liaise with relevant agencies/authorities/patients' families.
  • Ensure all referrals are attended to by relevant personnel.
  • Liaise with nurses on patient-related matters.
  • Supervise the operational work carried out by customer care coordinators when handling subsidy cases.
  • Act as a channel of communication between management and customer care coordinators.
  • Handle difficult customers and communicate escalated issues to the Manager.
  • Keep up-to-date records of patients (electronic or written data).
Required Skills and Qualifications

To be successful in this role, you will need:

  • Minimum O Level education.
  • Minimum 2 years of relevant working experience.
  • Expertise in Social Work and/or Customer Service would be an advantage.
  • Able to work on alternate Saturdays.
  • Working Location: Clementi/Changi.
