Senior Patient Care Specialist

We are seeking an experienced and skilled professional to provide high-quality patient care in various settings.
About the Role:
- The ideal candidate will have a strong foundation in nursing principles, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:
- Assess and take comprehensive histories of patients under their care.
- Perform physical examinations of patients under their care.
- Develop and implement individualized plans of care.
- Perform follow-up reviews of patients under their care; coordinate and evaluate care using appropriate care plans.
- Perform procedures within the scope of duties.
Required Skills & Qualifications:
- Patient education: Provide specialized patient education and training.
- Implement and coordinate individualized and group learning programs with patients and caregivers.
- Evaluate effectiveness of patient education; perform health education and discharge teaching to patients and families.
- Communicate treatment plans with patients and families.
Infection Control:
- Practice standard precautions when interacting with patients.
- Assist in monitoring and upholding infection control practices (e.g. standard precautions and hand hygiene).
- Educate patients and relatives on infection control awareness and practice.
Requirements:
- Valid SNB & BCLS certification.
- At least 2 years of ward/homecare experience.
