Job Title: Clinical Instructor

Job Overview

This role is responsible for implementing patient care in collaboration with multidisciplinary teams and ensuring timely discharge planning.

The successful candidate will have a minimum degree in nursing and at least 5 years of relevant inpatient and senior nursing experience.


Key Responsibilities
  1. Implement patient care in collaboration with the multidisciplinary team and according to assessed needs and health conditions.
  2. Facilitate admissions, transfers, and discharges; ensure timely discharge planning upon patient admission and appropriate to condition.
  3. Evaluate patients' progress according to planned care and readiness for discharge.
  4. Collaborate with the multidisciplinary team to deliver evidence-based care to patients to achieve desired care outcomes.
  5. Monitor patients for changes in condition, recognize early signs of clinical deterioration using appropriate physiological assessments, and escalate as required to enable timely implementation of interventions.
  6. Participate and contribute actively in multidisciplinary rounds, execute physicians' treatment and investigation orders promptly.
  7. Prepare and assist physicians with bedside patient procedures and treatments.
  8. Communicate and discuss patients' progress and response to treatment in collaboration with the multidisciplinary team.
  9. Demonstrate clinical skills, knowledge, critical thinking, and reasoning in the delivery of patient care.
  10. Assist the nursing team to problem solve if there are any patient or clinical issues and seek appropriate assistance/escalate to Senior Nurse Manager (SNM)/Nurse Manager (NM).
  11. Deliver patient and/or family education on disease progression, prevention, and rehabilitation and assess understanding of condition to enable participation in self-health care management.

Required Skills and Qualifications
  • Minimum degree in nursing with at least 5 years of relevant inpatient and senior nursing experience.
  • Advanced/Specialist Diploma in Geriatric/Community Nursing & Long-Term Care settings preferred.
