Key Responsibilities:- Provide case management services to patients admitted to the ward and discharged
- Track patients with high-risk behaviors or at risk of becoming revolving door patients and/or other special needs
- Provide follow-up case management in the outpatient setting
- Offer supportive therapy, counseling, and psychoeducation
- Conduct home visits as needed
- Degree in Psychology or a related field
- Experience working with individuals suffering from psychiatric disorders
- Strong team player with a passion for patient care, willingness to learn, and ability to contribute to the team
