Senior Care Coordination Specialist

We are seeking a highly skilled Senior Care Coordination Specialist to lead our Integrated Community Care Provider (ICCP) team in the Toa Payoh sub-region. This is an exciting opportunity for a motivated and experienced professional to make a meaningful difference in the lives of individuals and families.

The ideal candidate will possess a strong background in healthcare coordination, community care, or a related field, with at least 5 years of experience in managing partnerships and leading teams. A Bachelor's degree from an accredited University in Social Work, Healthcare Management, Gerontology, or a related field is required.

The Senior Care Coordination Specialist will play a vital role in aligning goals, establishing roles, and developing strategies and workplans for the Toa Payoh sub-region. Key responsibilities include:

  • Leading the development and implementation of care coordination initiatives across ICCP partners and external stakeholders.
  • Assessing resources and identifying opportunities for collaboration and integration.
  • Engaging and building networks and partnerships with other community care providers, regional health systems, grassroots organisations, and other stakeholders.
  • Representing the sub-region at meetings convened by AIC as required.

Additional responsibilities include collecting and consolidating information for quarterly progress reports, ensuring timely submission of endorsed reports and claim forms.

Requirements:

  • Bachelor's degree from an accredited University in Social Work, Healthcare Management, Gerontology, or a related field.
  • At least 5 years of experience in healthcare coordination, community care, or a related field.
