About the Role

As a Senior Care Coordination Specialist, you will play a vital role in supporting our community through effective coordination of care services.

Key Responsibilities
  • Lead cross-functional teams to implement sub-regional workplans and monitor progress.
  • Build relationships with partner organisations, regional health systems, and stakeholders to drive care integration and coordination.
  • Assess resources and identify opportunities for collaboration across partners.
  • Represent the sub-region at meetings convened by AIC and report back to leadership teams.
Required Skills and Qualifications
  • Bachelor's degree from an accredited University, preferably in Social Work, Healthcare Management, Gerontology, or related field.
  • At least 5 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships.
  • Strong understanding of community care delivery, integrated service models, and inter-agency collaboration.
  • Strong leadership, communication, and interpersonal skills, with the ability to build relationships and lead cross-functional teams.
  • Strategic thinking with an ability to align operational priorities with long-term strategic goals.
