Comprehensive Care Coordinator

This is an exciting opportunity to work with our organisation as a Comprehensive Care Coordinator. As a key member of our team, you will play a vital role in providing high-quality care to our clients in their homes.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with clients, caregivers, and healthcare professionals. You will also possess strong organisational and time management skills, with the ability to prioritise tasks and manage multiple projects simultaneously.

In this role, you will be responsible for conducting thorough assessments of clients' needs, developing individualised care plans, and facilitating engagement in adherence to the care plan. You will also perform nursing assessments, act as a general wellness monitor for the client, and communicate any wellness concerns to the client, primary caregiver, or doctor if necessary.

To be successful in this role, you will require a minimum of 3 years of relevant nursing experience in general or private hospitals. You will also need a Diploma in Nursing or similar, plus licenses to practice nursing in Singapore. Experience in-home nursing and/or Dementia Care is preferred but not mandatory.

Key Responsibilities:

Qualifications:
