Job Title: Medical Social Worker

Overview:

The role of a medical social worker involves providing comprehensive support and services to residents and their families. This includes assessing psycho-social needs, developing care plans, and facilitating communication between residents, family members, and healthcare professionals.

Key Responsibilities:

  • Attend multidisciplinary meetings to discuss and coordinate care plans.
  • Design and develop programmes that promote quality care and well-being.
  • Counsel residents and their family members on psycho-socio care plans.
  • Liaise with external organisations to meet residents' and their families' needs.
  • Focus on family-centred care by encouraging participation among residents' family members.
  • Participate in ward visits and organisational events.
  • Identify gaps and streamline work processes within the organisation.
  • Advocate for changes to enhance the care and well-being of residents.
  • Stay up-to-date with government guidelines on healthcare and social service programs.
  • Develop relationships with internal colleagues to foster close liaison.
  • Conduct pre-admission assessments as part of the MCAT team.
  • Establish and confirm source of social facts shown in the social background of new applicants.
  • Manage Medifund administration and applications in collaboration with Finance Department.
  • Administer renewals of residents' funding schemes on a timely basis.
  • Develop and monitor the social wellness care plan of residents.
  • Develop discharge care plans for residents if their health condition is fit for discharge.

Requirements:

  • Possess a Degree in Social Work.
  • At least 1-3 years of relevant healthcare industry experience.
  • Exhibit a pleasant and mature disposition with empathy and patience dealing with residents, next-of-kins, and staff alike.
  • Demonstrate initiative to identify, plan, develop, and implement resourcefulness to find solutions to missing gaps in work processes and mutually benefit stakeholders.

  • Excellent communication and people engagement skills.
  • Good organisational ability.
