Lead Community Care Coordinator

Job Summary:

  • Oversee a team of care coordinators to provide post-discharge care coordination and case management services, ensuring optimal health outcomes for patients.

Key Responsibilities:

  • Develop and implement policies and procedures to maximize support for patients and caregivers.
  • Lead the care coordination team in conducting post-discharge follow-up to ensure patients and caregivers' ability to cope with community services and caregiver education.
  • Manage data entry, data management, and analysis to inform program development and evaluation.
  • Negotiate with healthcare partners to provide continued support post-discharge from the hospital.

Qualifications:

  • Professional qualification in nursing, social work, or allied health, or other relevant clinical disciplines.
  • At least 8 years of experience as a care coordinator, case manager, or community nurse in a community healthcare setting.
  • 3-5 years of leadership experience managing a team.
