
Community Outreach Specialist

This position involves providing support services to individuals during the aftercare phase of a program. As a member of our team, you will work closely with the Incare team to deliver integrated services as part of a community-based initiative.
Duties and Responsibilities
- We are seeking a candidate who can assist in conducting pre-emplacement interviews with residents to understand their needs and work with them to set goals.
- The successful candidate will prepare and maintain Individualised Service Plans (ISPs), progress reports from incare to aftercare phases, and provide administrative support for basic intervention tasks identified by social workers/managers.
- You will also be responsible for managing complaints, issues, concerns or requests from residents to Social Workers/Managers when necessary.
- Motivate residents through day-to-day interaction and ensure daily needs are well taken care of.
- Monitor workplace safety and health standards.
- Provide admin, operational and logistic support in planning and conducting programme activities for residents and staffs in both incare and aftercare phases.
- Support with administration, operational and engagement of volunteers.
Community Relations and Outreach for AIMS
- Deliver community relations activities and initiatives for its alumni group including WISDOM.
- Plan and execute community engagement activities, events and communications for AIMS, liaise with community leaders and volunteers.
- Develop partnerships with community agencies that supports or complements the mission and interests of AIMS.
- Execute media and communication duties in partnership with HCSA's Strategic Communications.
- Lead or support volunteer activities relating to community & corporate engagement activities and events.
Resource Development and Volunteer Coordination
- Identify and engage potential partners who can contribute to AIMS operations and maintain communication lines.
- Coordinate resources and activities from the community that supports AIMS and its initiatives.
- Ensure effective overall administration and sound fiscal management of donations/funds restricted to AIMS.
- C coordinate the Educate, Test and Treat teams of volunteers to provide support and interactions with fellow Halfway House partners.
- Organise and oversee the growth of AIMS community and engaging them in advocacy and volunteerism.
- Work with the Aftercare Manager to support the volunteer relationship system from recruitment to appreciation.
- Support maintaining records of volunteers' information from AIMS, assignments and contributions for reporting and analysis.
