- Capture accurate call information in our system and electronic patient records.
- Follow up on clinical calls after two weeks to review patient outcomes.
- Participate in activities contributing towards patient care improvement, including professional development sessions.
Key Responsibilities:
- Medical Triage: Respond to complex medical queries from patients and caregivers via our 24/7 telephone hotline.
- Call Information Management: Capture and record detailed information regarding each call, ensuring accuracy and completeness.
- Follow-Up Calls: Conduct follow-up calls with patients two weeks post-call to assess outcome and gather feedback.
- Patient Care Improvement: Engage in continuous learning and skill-building opportunities to enhance patient care delivery.
