Clinic Care Coordinator

We are seeking a skilled and compassionate Clinic Care Coordinator to join our team. As a Clinic Care Coordinator, you will be responsible for coordinating and providing patient care activities, utilizing IT systems to track and document established clinical indicators and care given to patients.

Key Responsibilities:

  • Assisting in coordinating and providing patient care activities.
  • Using IT systems to track, perform and document established clinical indicators and care given to patients.
  • Performing basic parameter measurements, closing preventive screening and vaccination gaps in relation to national programmes/initiatives.
  • Providing basic health education to improve patients' knowledge on lifestyle management, social and physical activities through social prescribing, the use of health monitoring devices e.g. home blood pressure, glucose and weight monitoring, as well as advising patients on vaccination requirements.

Required Skills and Qualifications:

  • Diploma in Health Promotion (advantageous).
  • Prior healthcare experience (advantageous).
  • Good verbal and written communication skills.
  • A team player with good interpersonal & communication skills.
  • Passionate to work in Healthcare industry.
  • Customer-oriented, a team player, a fast learner and serious with work.
