We are seeking a highly skilled and experienced professional to join our team as a Healthcare Network Development Manager at AGENCY FOR INTEGRATED CARE PTE. LTD.

Key Responsibilities
  1. Collaborate with Healthcare Partners: Foster strong relationships with hospitals and community partners to identify operational gaps and develop solutions to address them.
  2. Develop Referral and Care Processes: Design and implement processes to enable the flow and escalation of clients across care settings, supporting community partners in caring for individuals with mental health conditions or complex needs.
  3. Conduct Ongoing Review and Development: Continuously review and develop new models of care to support clients requiring mental health care support.
  4. Manage ASCAT and Aftercare Programmes: Plan, develop, implement, monitor, and review the ASCAT and Aftercare programmes, including enablers such as funding and processes to support the development of the community health ecosystem.
  5. Present Findings and Strategies: Present findings and potential strategies to senior management, government stakeholders, and industry players to secure buy-in for execution.
Key Skills and Qualifications
  • Leadership and Management: Manage and lead a group of programme managers, providing supervision, guidance, and advice to assigned team members.
  • Communication and Facilitation: Develop platforms for discussion and cross-learning with staff on programme management, and facilitate collaboration with diverse groups.
  • Business Acumen and Analytical Skills: Analyze programme care models, propose funding models, and manage programme key performance indicators, including budget management.
  • Education and Experience: Bachelor's Degree/Postgraduate Diploma/Professional Degree in Social Work/Psychology/Counselling, with at least 8 years' experience, including 2 years in a managerial capacity, in healthcare or community care sectors.
