Job Overview

We are seeking a dedicated and compassionate individual to join our team as a Home Healthcare Nurse. In this role, you will play a vital part in supporting clients and their caregivers through the transition from hospital to home.

 

About the Role

This is an excellent opportunity for a skilled nurse to provide holistic support services and make a meaningful difference in the lives of our clients.

  • Conduct comprehensive assessments of potential clients and their home environments.
  • Develop and implement individualized care plans, working closely with caregivers and family members where possible.
  • Monitor clients' conditions and regularly review and update care plans to ensure optimal outcomes.
  • Provide financial counseling and guidance on government subsidies available to eligible clients.

 

Key Responsibilities

In this role, you will be responsible for:

  • Providing customer-centric support services to clients and their caregivers.
  • Coordinating seamless transitions from hospital to home care.
  • Developing and implementing effective care plans that meet the unique needs of each client.
  • Monitoring client progress and making adjustments to care plans as needed.
