Job Description

As a Psychosocial Care Specialist, you will play a pivotal role in delivering holistic care across various healthcare settings. Your responsibilities will include conducting comprehensive assessments, providing counseling and emotional support, collaborating with the multidisciplinary team to develop individualized care plans, and advocating for patients' needs.

  • Conduct thorough psychosocial and financial assessments for patients and their caregivers.
  • Deliver direct counseling and psychoeducation to facilitate coping with illness, grief, and life transitions.
  • Collaborate with the MDT to develop personalized care plans and facilitate family conferences.
  • Act as an advocate and liaison for patients and their families, ensuring access to necessary resources and services.
  • Maintain accurate documentation of assessments, interventions, and progress notes.
  • Participate in quality improvement initiatives and contribute to departmental development.
Required Skills and Qualifications

  • Master/Bachelor/Post Graduate Diploma in Social Work.
  • Registration with the relevant professional association.
  • Excellent interpersonal skills and ability to communicate effectively at all levels.

  • At least 3 years of relevant experience in healthcare or social care settings.
