Job Title: Residential Care Coordinator

Overview:

This role involves coordinating comprehensive psychosocial assessments and developing intervention plans to address residents' emotional, financial, and social needs. The candidate will work closely with the care team to integrate social perspectives into holistic resident care.

  • Conduct thorough psychosocial assessments and develop targeted intervention plans to meet residents' complex needs.
  • Foster open dialogue between residents and their families to facilitate advance care planning and financial assessments.
  • Perform crisis and risk assessments, and intervene promptly to maintain a safe care environment.
  • Coordinate discharge processes, linking residents with community services that support their continued well-being.
  • Provide empathetic individual and family counseling to build resilience and strengthen coping mechanisms.
  • Participate actively in multidisciplinary team discussions to ensure seamless integration of social perspectives into resident care.
  • Maintain accurate case documentation and data entry in client management systems.

Requirements:

The ideal candidate will possess:

  • A degree in Social Work or Postgraduate Diploma in Social Work from a recognized institution.
  • Registration with the Singapore Association of Social Workers (SASW) is highly desirable.
  • 1-2 years of relevant social work experience in healthcare, eldercare, or community settings.
  • Experience in casework, counseling, or care coordination is highly valued.
