Job Title: Wellness Coach

We are seeking a skilled Wellness Coach to join our team. As a Wellness Coach, you will play a vital role in delivering holistic care to patients and promoting their overall well-being.

Key Responsibilities:

  • Patient Care: Develop and implement wellness initiatives and activities at the community dialysis center, ensuring continuity of care plans for each patient.
  • Screenings and Assessments: Conduct routine reviews to track patient adherence and progress in exercise programs, and provide recommendations for follow-up care.
  • Equipment Maintenance: Ensure proper maintenance, disinfection, and servicing of exercise equipment to promote a safe and healthy environment.
  • Collaboration: Work collaboratively with the Clinical Services Team to identify and address barriers to participation, ensuring patients receive adequate support in their wellness journey.
  • Volunteer Events: Support the planning and execution of volunteer events and social outings to empower senior patients.

Requirements:

  • Prefarable Diploma in Health Sciences or other relevant courses (e.g., sports sciences, therapy aide, community care associate).
  • Relevant working experience in a healthcare setting is advantageous.
