
community eldercare executive

Job Descriptions:
1. Conduct home visitation to Eldercare Clients together with team members.
2. Assess clients' needs and raise referrals for clients to appropriate social services provider.
3. Follow-up with and monitor feedback of clients, to ensure recommended services are in place and aligned with
the needs of clients, families and caregivers.
4. To assist Community Eldercare Manager on planning & execution for all recreational activities for the clients.
5. Participate in regular client review discussions with other team members and recommend modification for
better services.
6. Provide fast response to clients' needs.
7. Data entry & ensure timely accuracy reporting to Manager & Authority.
8. Maintain accurate documentation of client information, and ensure the Manager & relevant agencies
are kept updated of changing client needs.
9. Other duties as assigned by Manager.
Requirements:
1. Entry level welcome.
2. Be prepared to be available for activities during the non-office hours.
3. Team player in a multi-disciplinary team setting with ability to perform basic social screening.
4. Strong interpersonal and people skills.
5. Passion for working with the elderly and conducting home visits.
