•Assist and support Programme Director, Programme Manager and the respective Clinician Leads in the implementation, development and management of the programme.

•Coordinate care across the various care settings and act as patient advocate between patients, caregivers and hospital (including telephonic follow-up post operation).

•Organise regular multidisciplinary team meeting (including case discussion) and ensure appropriate documentation at all time.
