Senior Assistant Programme Executive – Integrated Community Care Provider (ICCP) Admin Point Of Contact (POC) – Dept Role overview

The Admin POC is a dedicated resource to support the organisation with working through all the relevant aspects of the partnership model used to operate the ICCP in the sub-region. The Admin POC supports partner organisations in the sub-region to build strong partnerships, align goals, establish roles and practice collaborating and working as a “team” to achieve the ICCP objectives.

Responsibilities
  • Secretariat and Coordination Function
    • Work with leadership teams of partner organisations to align goals, establish roles, and develop strategies and workplans for the sub-region to achieve the deliverables specified for Phase 1 and 2.
    • Work with centre managers and ground teams to implement the sub-regional workplan, and monitor and report on progress.
    • Organise and facilitate regular meetings and check-ins for centre managers and ground staff to build understanding and address challenges.
    • Engage and build networks and partnerships with other community care providers, regional health systems, grassroots organisations and other stakeholders.
    • Collaborate with ICCP providers to trial joint referral workflow in Phase 1 to support care coordination deliverables in Phase 2.
    • Collaborate with the AIC’s sub-regional Relationship Manager on reporting and monitoring of indicators.
  • Administrative Function
    • Collect and consolidate information required for the quarterly progress report.
    • Represent the sub-region at meetings convened by AIC as required, and report back to the leadership teams.
    • Ensure endorsed reports and claim forms are submitted to AIC in a timely manner.

  • Able to multi-task and work independently with minimal supervision
