Job reference: Community Care Associate (CCA) – 5 days work week, Monday to Friday. Working hours: 9 am to 6 pm. Able to travel around Singapore.

Responsibilities
  • Provide Home Based Therapy and Intervention and assistance to clients in their daily living, following care plans developed by social service and/or healthcare professionals under Community Based Support Services (CSS).
  • Maintain up-to-date documentation of clients\' conditions and provide inputs for the development and review of care plans.
  • Conduct activities for clients to promote independence, health, wellness, and quality of life; guide clients on personal and social skills, self-care and community living skills.
  • Provide client training and support; conduct programmes and activities; communicate effectively with caregivers and work on training plans.
Qualifications & Skills
  • Minimum ITE certification.
  • Experience in education/training of persons with intellectual disability or in a multi-disciplinary team environment is an added advantage.
  • Self-motivated and able to participate in on-site and off-site activities.
