Join Us to be our Palliative Care Coordinator, where you'll be the guiding light for patients and families navigating complex healthcare journeys. You'll wear many hats – part navigator, part educator, part data wizard, and full-time compassionate human being. You stand to gain immense satisfaction from direct patient care delivery with key contributions to efficient and timely care coordination including diagnosis, treatment, rehabilitation, follow-up, survivorship, and end of life care.

Responsibilities
  • Direct care and care navigation; provide individualized assistance (informational and emotional support) to patients and their families to overcome healthcare system barriers, improve continuity of care and quality of palliative care.
  • Identify and navigate high-risk patients to ensure access to resources; follow up with patients by phone where necessary to reduce unnecessary ED visits and admissions.
  • Follow up with patients who defaulted on clinic visits to ensure continuity of care.
  • Assist patients and families to transition across services and locations; communicate effectively with healthcare professionals to ensure smooth transitions.
  • Perform assessments to identify physical, psychosocial and spiritual needs and barriers to accessing care; initiate interventions to address these needs.
  • Conduct outcomes evaluation of care pathways.
  • Coordinate care across services and settings; ensure compliance with palliative care pathways; liaise with community partners; refer appropriately to specialists, allied health and support services.
  • Liaise with internal stakeholders (Medical Oncologists, Surgical Oncologists, Radiation Oncologists, Radiologists, etc.).
  • Engage in education of patients and families on palliative care approach, symptom management and available resources; triage patients to subspecialty care.
  • Support clinic staff with internal referrals, community home care & day care referrals, and financial counselling estimates; participate in data analytics and quality improvement projects; measure outcomes with VDO teams.
  • Coordinate palliative care research and quality improvement projects; ensure compliance with research ethics and data governance policies.
  • Collaborate on national initiatives with SHC and MOH; participate in workgroups or pilot programmes.
  • Facilitate quality improvement activities and assist in expanding palliative care coordination and navigation services; participate in strategic planning.
  • Any other projects/tasks as assigned by the department.
Qualifications
  • Bachelor degree in Health Sciences; degree in Nursing or Allied Health relevant to the job preferred.
  • Candidates with relevant working experience in a similar setting preferred.
  • Excellent interpersonal and communication skills; ability to work independently and as part of a team.
