**What the role is**
- The Social Policy and Services Group (SPSG) oversees all policy, planning and delivery of social services for low-income and vulnerable families, including persons with disabilities. The Service Delivery and Co-ordination Division (SDCD) under SPSG overseas direct operations including the Social Service Offices, centralised schemes and service management of regional services. You will be part of a dynamic team tasked to support the Social Service Offices.

**What you will be working on**
- Based in the Social Service Office (SSO), the UPLIFT TLC will plan and operationalise the outreach and programme support for students who are identified for the UPLIFT Community Pilot. He/she would be coordinating support from Government agencies and Social Service Agencies (SSA) for target students and their families, organising outreach through volunteer befrienders to target families, monitoring student and family progress, and networking between schools and community partners to support development of community. Provide Case Oversight - Collate and consolidate triage data of students and families, based on engagement with families and information from schools, SSAs and family befrienders. - Maintain a list of programmes relevant to target students and their families. - Match students and families to relevant sources of community support based on needs. - Maintain records of school and community touchpoints of target students and family, e.g. school POCs, or Family Service Centres (FSC) - Work with existing target students/families, schools and community touchpoints to support target students and families - Coordinate external referral to community support programmes or other support schemes and services where necessary, and monitor the outcomes and progress of referrals - Work with schools and community agencies to monitor target students’ progress Manage Volunteer Befrienders - Support MOE Uplift Programme Office (UPO) on volunteer recruitment and selection - Collaborate with UPO on volunteer training (including locale-specific training or specialised training if necessary) - Deploy volunteers to suitable families for befriending (first contact with families will be done with the TLC) - Provide on-the-job support to volunteers, incl. regular check-ins Develop of Community Resources and Support - Plan engagements and facilitate partnerships between local organisations, including schools, social service agencies, and grassroots - Assess presence of service gaps from unmet needs of target student and family for joint service planning with SSAs, SSO’s Regional Services Team or UPO
