Singapore National Healthcare Group Full time

-Family Group: Administration**STATEMENT OF PURPOSE**:
The Population Health Management (PHM) Directorate has been established to support daily clinical management to become more systematic and targeted; to ensure NHG is more informed by clinical, operational and demographic data as Regional Health Manager for Central and North Singapore; and that this informs the way in which we deploy our collective resources across all organisations to maximise the health outcomes of the local population, and establish system planning and care delivery so it is more coherent and systemic in its understanding of population health outcomes, value, and equity.

Our role in the PHM Directorate is to facilitate this change across NHG at population, provider, place, and resident level - ensuring that we address not just the clinical and preventative aspects of care but also the social, environmental, and behavioral determinants of health - so that we can achieve our ambition to add 15M health years for our residents.

This means that the Population Health Management and Value Directorate is principally an ‘enabler’ of this operational and cultural change - developing enabling infrastructure and building new capability at every level of our system in NHG on behalf of our partners and population.

- **Profiling and Segmentation**: Drive population profiling and segmentation to inform system planning and care delivery to become more systematic and targeted, and
- **Population Health Research and Evaluation**: Use advanced data analytics to generate actionable insights, enabling researchers to identify trends, evaluate interventions, and prioritize areas for population health improvement.

- **Population Health Data Mart**: Harness understanding of healthcare data systems and databases to guide the development of a population level data mart that facilitates efficient analysis and reporting for population health.

**Key areas of responsibility**:

- Leverage multiple data sources to collect, integrate, analyze, and visualize data to effectively profile and segment our resident population.
