**Work location**:
Lentor Avenue (Next to Bullion Park Condominium)
**Duties & Responsibilities**:
- Assess the strength and resilience of the resident, families, and social support systems to help the individual function within the community
- Co-ordinate social services through a case management approach
- Identify and refer cases for financial assistance
- Financial counselling
- Address psychosocial needs through care and counselling
- Educate resident’s family member on the physical and psychosocial needs and mediate familial conflicts
- Assist staff members in understanding the significant social and emotional factors related to the residents’ health problems
- Assess risk of self-harm and to others
- Work with nursing and other allied health professionals, residents and their families to develop and implement the care plan and the post discharge care plan
- Discuss long-term care issues with the resident and family
- Train other staff members on communication skills, mediation skills, stress management etc
- Other duties assigned by the Management
**Requirements**:
- Degree/Post Graduate Diploma in Social Work with relevant experience
- Minimum 2 years of relevant experience
- Registered Social Worker with Singapore Association of Social Workers
- Good interpersonal and communication skills
