The Medical Social Worker in Ju Eng Home, you will play a crucial role in providing essential support and advocacy for residents and their families. Your responsibilities will revolve around addressing the psychosocial needs of residents, collaborating with interdisciplinary teams, and ensuring access to necessary resources and services to enhance their quality of life.

**Key Performance Area
**1. Assessment and Care Planning**:

- Conduct comprehensive psychosocial assessments of residents upon admission and regularly thereafter.
- Identify emotional, social, and environmental factors impacting residents' well-being and health outcomes.
- Collaborate with the interdisciplinary team to develop individualized care plans that address residents' psychosocial needs.

**2. Counselling and Support**:

- Provide counselling and emotional support to residents and their families, addressing issues such as adjustment to long-term care, grief, and coping with illness.
- Offer guidance and assistance in navigating complex healthcare systems, including financial concerns.
- Facilitate support groups and educational sessions for residents and families on relevant topics.

**3. Advocacy**:

- Advocate for residents' rights and preferences, ensuring their voices are heard in care planning and decision-making processes.
- Collaborate with residents, families, and staff to resolve conflicts and address concerns related to care and services.
- Act as a liaison between residents, families, healthcare providers, and community agencies to facilitate communication and access to resources.

**4. Discharge Planning and Transition Support**:

- Coordinate discharge planning processes to ensure smooth transitions for residents returning home or transferring to other care settings.
- Assess residents' support systems and environmental factors to determine appropriateness of discharge plans.
- Provide referrals and linkages to community resources, including home health services, transportation, and social support networks.

**Documentation and Reporting**:

- Maintain accurate and timely documentation of assessments, interventions, and discharge plans in residents' medical records. Prepare reports and documentation required for regulatory compliance and reimbursement purposes.

**Requirements
**Qualification**:

- Degree in Social Work from an accredited institution.
- Registered social worker with Singapore Association of Social Workers

**Experience Level**:

- Previous experience in a healthcare or long-term care setting, preferably working with elderly.
- Strong assessment, counselling and communication skills
- Knowledge of relevant regulations, ethical standards, and best practices in social work.
- Ability to work collaboratively in a multidisciplinary team environment.
- Compassion, empathy, and a commitment to advocating for vulnerable populations.
