The Senior Associate is responsible to support and to coordinate the administrative clinical care for renal patients in the inpatient and outpatient setting.

**MAJOR DUTIES AND RESPONSIBILITIES**:
**(A) SPECIFIC**

1. To coordinate Holistic Approach in Lowering & Tracking CKD (HALT-CKD) patient referrals and data entry.

2. To assist with the coordination of group patient education programmes.

3. To coordinate pre-PD home visit with peritoneal dialysis (PD) vendors.

4. Maintain and update PD patient records, lab results and treatment plan.

5. To schedule and coordinate outpatient dialysis placement by providing the necessary documents required of the dialysis centre.

6. To schedule outpatient vascular access creation appointments.

7. To ensure timely submission of documents in BRIGHT for voluntary organisation welfare (VWO) HD centre.

8. To submit monthly HD and PD patient statistics to National Registry Disease Office (NRDO).

9. To track dialysis patients’ admissions and discharges specific to renal care.

10. To obtain the necessary clinical and administrative information for audit and quality improvement for procedures done by nephrologists.

11. To collate data for yearly department report and presentation.

12. Research administrative duties and coordination of patient research visits.

**(B) GENERAL**

1. Administrative, data collection and document accurate minutes of meeting.

2. Perform any other ancillary duties as assigned by the reporting officer.

**EDUCATION, TRAINING AND EXPERIENCE**
- Higher Nitec / ‘A’ Level.
- Proficient in Microsoft Office.
- Excellent communication, time management, and interpersonal skills.
- Possess good interpersonal and communication skills.
- Able to work independently.
- Strong attention to detail and accuracy in handling sensitive patient information.
