**Position**:Care Coordinator, Psychological Medicine
The Care Coordinator is primarily responsible for liaising with various stakeholders, such as family members, doctors, Allied Health staff and external agencies (AIC, Day Care Centres, schools, family care centers etc.). He/She is expected to assist patient care in the clinic and facilitate patient care pre / post clinic through telephonic and/or face-to-face interactions.

**Responsibilities and Duties**
You will be responsible for the following**:
- Collect basic socio-demographic information for patients assigned by reporting officer.
- Contact patients prior to their first visits to confirm their attendances and remind them to attend their first visit and follow up appointments on time.
- Triage cases based on their clinical needs under the supervision of a psychiatrist.
- Provide psycho-education for patients and their families/caregivers.
- Assess caregivers’ needs with appropriate assessment tools and arrange appropriate referrals for support.
- Collaborate with allied health staff such as medical social workers, psychologists, occupational therapists, and pharmacists for the management of patient’s assessed needs. Includes AIC, Day Care Centers, Schools, Family Care Centers etc.
- Monitor patient’s appointments, investigations, and social and rehabilitation plans.
- Facilitate discharge to step-down care (polyclinics, GP clinics, SLH etc.) once diagnosis and care plans are finalized.
- Ensure that relevant patient education materials are available and are up to date.
- Cross cover for other patients who need case management.
- Contact patients and the family prior to their first visits for Lasting Power of Attorney (LPA) to confirm their attendances and remind them to attend their first visit and follow up appointments on time.
- Perform ECG, blood taking, memory assessment and others (if any) for patients _(training to be provided)_
- Perform data management, collating and maintaining of patient database for clinical and research purpose.
- Assist in tracking, maintaining, and reporting of quality clinical indicators and provide statistical report and updates on a regular basis.
- Participate in projects related to case management and clinical pathway.
**Job Requirements**:
**Qualification**:
- Degree in Nursing/Health Science.
**Experience**:
- Prior relevant work experience is preferred.
- Good knowledge of geriatric, adolescent/child care and community skills.
- Proficient in Microsoft Office suite.
- Able to work in multi-disciplinary teams and to work independently.
- Excellent interpersonal and communication skills.
