DESIGNATION : Transition Coordinator

RESPONSIBILITIES
The Transition Coordinator (TC) holds managerial and clinical responsibility in overseeing the transition of clients from SPED and the community into post-18 services and programmes. This role involves collaboration with clients, their families, services and programmes as well as other community/government stakeholders to facilitate transition planning pre and post service so that the clients benefit from person-centred outcomes.

**Transition Planning from SPED/community into post-18**
- Work closely with SPED Transition Planning Coordinators (TPCs) and SG Enable (SGE) to manage referral matters of graduating students and clients from the community, respectively.
- Work closely with Allied Health Professionals on the management of transition needs of clients' pre-admission and post-discharge out of MINDS services.
- Leads Case Conferences and manage the progress of clients who are discharged out of MINDS services so that their service needs are met post-discharge and that they and their families continue to be checked on over time.

**Coordinate transition planning at a cluster level to achieve person and family-centric outcomes.**
- Lead Transition Support Officers across the cluster on Individualised Development Plan (IDP) progress of clients.
- Plan and oversee the Transition planning needs of clients in the cluster.
- Manage client IDP data analyses, reporting and dashboarding.

**Maintain networks with relevant post-18 services providers.**
- Work closely and manage networks with key stakeholders from SPED schools, SGE, and other community partners
- Develop and review processes and SOPs to track development and refinement of transition processes and aligned across sectors, as required.

**Support capacity building of TSOs in implementing effective client Transition Planning**
- Guide, groom, supervise and appraise TSOs
- Organise training and learning journeys for TSOs to build their person-centred practice and planning competencies.

QUALIFICATIONS
- Degree in Social Work, Psychology, Community Services or other relevant fields with at least 5 years working experience in the disability sector.

OTHER INFORMATION
- A strong team-player with excellent people skills.
- Competencies in person-centred planning/practice.
- Possesses strong organisation skills, time management and problem-solving skills.
- Possesses strong written and verbal communication skills
- Ability to speak in one additional local language apart from English an advantage
- A self-motivated and adaptable individual.
- Ability to work well both independently & in lead a team with integrity, sensitivity, knowledge and drive to support & advocate for their clients with intellectual disability.
