Rehabilitation Counsellor

**JOB SUMMARY
To provide support and care for residential and day care clients who are persons with mental health conditions in their journey towards recovery, community re-integration and an improved quality of life. The Rehabilitation Counsellor will assess clients' needs and design rehabilitation programmes that may include vocational counselling, training and job placement.
**KEY DUTIES AND RESPONSIBILITIES**
- Care and provide support for 30 residential / day care cases
- Provide rehabilitative counselling, initiate referrals, organize family sessions, and make home visits when required
- Interview and assess individuals seeking admission to the Centre
- Attend Review and Admissions Committee meetings, case consultations, case conferences, staff meetings
- Conduct rehabilitative programmes such as skills training and mental wellness activities
- Explore and implement transition / discharge plan for cases recommended for discharge
- Maintain proper and up-to-date documentations and records for services provided to cases assigned
- Assist in other duties assigned by the Manager and/or Centre Director
**REQUIREMENTS**
- Degree in Counselling, Psychology, Social Work
- 2-3 years of relevant experience
- Excellent interpersonal and communication skils
- Good listening skills, patient, compassionate
- Strong relational skills and able to build rapport quickly with distressed people
- Knowledge in psychiatric rehabilitation and case management
- Good teamplayer
