To provide case management services, provide emotional support and psychoeducation to youth and caregivers, establish appropriate service linkages, and work towards providing holistic care support to those living in the community who are at risk of depression, anxiety and other mental health conditions.
- To make regular phone calls and make periodic invitations to suitable clients for participation of Youth’s programmes and activities.
- To provide person-centric care (social and health), information, and support for youths and caregivers through emotional support, follow-up, and service linkage.
- To conduct wellbeing assessment screening for youths.
- To participate in mental health awareness outreach events to raise awareness of mental health to youth; promote the recognition of early signs and symptoms of mental illnesses.
- To provide ad-hoc support and training to colleagues in their case management activities.
- To ensure accurate and timely documentation and secure storage of case files, referrals, and other related documents in respective systems.
- To assist Youth Program Managers in the planning and executing of youth programs.
- To participate and support in ad-hoc excursion trips to build rapport with youths and their caregivers.
- Any other administrative roles that may be assigned to you.

Prior experience in handling or coordinating events and volunteers.
