Roles & Responsibilities

**Job Summary**:
Provide befriending and outreach services to elderly within designated service boundary. Work closely with the Centre Manager in implementing befriending service and monitoring of clients’ condition.

**Job Description**:

- Conduct community outreach and relevant need assessments to elderly within designated service boundary.
- Provide befriending services to frail and vulnerable elderly at their homes.
- Provide referral to help services for the elderly in needs.
- Recruit volunteer befrienders to assist with the outreach and befriending services.
- Train volunteer befrienders to provide monitoring and appropriate referral to support services.
- Perform administrative and reporting duties such as visitation notes, records and other necessary data and statistics required by the authorities.
- Perform other duties as assigned by the reporting officer.

Job Requirements
- Minimum Diploma Holder.
- 1 to 2 years of relevant experience in eldercare or social service.
