- Other- Work closely with the Satellite Offices (SOs) to set direction for and implement capability development initiatives for staff on matters pertaining to care and support, network development and community care.
- Develop and review internal and external processes across SOs and community partners to ensure proper care transition, care integration, coordination etc
- Develop and review care and support protocols and practice guides to ensure care processes and standards are established across SOs i.e. triaging, care assessment, coordination, intervention and monitoring.
- Support capability building and professional development efforts to ensure that the SO staff are trained to perform the above mentioned.
- Support overall network development efforts across government and non-government agencies to support organisational priorities focused on building our community care ecosystem
- Support the development of local networking building strategy and engagement plan to support network and community care ecosystem development.
- Gather ground feedback from the SOs and agencies to identify service gaps and drive the development of initiatives and interventions to meet seniors’ needs

**Job Requirements**:

- Degree in any discipline with at least 2 years of working experience preferably with knowledge and familiarity with the healthcare or social sectors
- Analytical and organised, able to translate concepts into actionable goals
- Confident in working with external stakeholders
- Good in communication, both verbal and written
- Meticulous, team player, resourceful and able to multi-task in a fast-paced environment
