**Overview**:

- Our Healthcare Institution client is looking for energetic and highly motivated Individual to join their fast-growing industry as Care Coordinator.
- Conduct necessary preventive health screenings and assessments, such as vision tests and blood glucose tests, based on individual patient needs.
- Identifying patients who would benefit from lifestyle modification education programs such as smoking cessation or weight management and refer or enrol them in care management programs that are tailored to their specific health needs.
- Comfortable with using systems as need to document and track patient progress / schedule their appointments for the programmes.
- Assist in in planning, organizing, and coordinating health promotion activities for staff members at the workplace, and will also be expected to carry out any additional duties assigned by your supervisor.

**Job Requirement**:

- Minimum GCE 'O' Level
- Qualification in health promotion will have an advantage.
- Experience in healthcare will be advantageous
- Self-motivated and has interest in patient care.
- Good communication and coordination skills
- Proactive and a team player
