**Key Reponsibilities**:

- Conduct means testing for clients.
- Network with various agencies and individuals to mobilize social resources to meet patient’s needs.
- Identify presenting or underlying issue and utilizes available resources to develop a case plan to enhance, maintain and promote psychosocial functioning.
- Identify more complex psychosocial or bereavement needs, and refer to psychosocial services (PSS) team when required.
- Conduct home visits when necessary.
- Advocate for the protection of patient’s dignity, confidentiality and

Participate in multi-disciplinary discussions and case conferences.

**Qualifications and Experience**

Degree, preferably related to psychology or social work

**Competencies **
- Good knowledge of various funds and schemes available to the patients with cancer
- Conduct client assessment
- Client interviews and home visits
- Positive attitude Good communication skills

**Attributes**
- Strong interpersonal skills
- Comfortable working with people of all levels Willing and able to work on weekends when there are events

**Languages Skills **
- English and a second language (ability to speak Chinese dialect will be beneficial as many clients speak on Hokkien or Teochew)

**Skills**
- Motivational interviewing techniques
- Case work management Means testing

**Computer & Software Literacy**
- CRM
- MS Office especially Excel
