About the RoleAbout the Role:
As a Care Planning Coordinator, you will play a crucial role in coordinating the AN-ACC process to identify and maximise funding opportunities across our Care Centre. You will work closely with management and care staff to conduct resident assessments, while also providing training and expert guidance to support the care team.
This position is part-time, requiring 2 days a week (16 hours per week) at IRT Woodlands in Meridan Plains.
Your work may include:
• Leading, planning, and coordinating the AN-ACC process, ensuring all documentation is submitted accurately and on time prior to assessments.
• Reviewing and updating clinical assessments (e.g., PAS, Braden Scale, Activities of Daily Living) to reflect changes in residents' health status.
• Participating in care evaluations, clinical meetings, and handovers to identify residents for reassessment.
• Actively contributing to the continuous improvement of services to optimise funding opportunities.
• Providing training and advice to Care Centre team members, fostering a culture of responsible assessment and documentation.
• Ensuring care assessments, care plans, and ongoing reviews are completed efficiently and accurately.
• Overseeing the supportive evidence for AN-ACC claims, ensuring compliance with validation and legislative requirements.
• Current AHPRA nursing registration (EN/RN), or a Certificate IV in Individual Support, Certificate III in Aged Care (with Medications unit), or a similar qualification.
• Previous experience in a similar role (preferably within Aged Care)
• A demonstrated understanding of the AN-ACC or prior funding models.
• Proven clinical, assessment and care-planning skills
• Strong interpersonal and communication skills.
• In-depth knowledge of clinical issues relevant to aged care.
• Sound problem solving, influencing, negotiating and conflict resolution skills
• A keen attention to detail and excellent analytical skills.
• Demonstrate a proficiency using computer systems operating in a Microsoft Windows
• environment including Microsoft Outlook, Excel and Word
