Case Management Officer - G-RACE (Contract)

This role involves working with the Multi-Disciplinary Team (MDT) to ensure holistic patient care by assessing needs, planning, coordinating, and monitoring interventions.

Primary Responsibilities and Duties
  1. Respond to inquiries regarding referrals to G-RACE Home Clinic or Allied Health Intervention (Home-Based).
  2. Arrange home visits with patients and families.
  3. Open case folders and conduct assessments during home visits.
  4. Establish rapport and provide home interventions and psycho-education.
  5. Develop and discuss individualized care plans.
  6. Update team members during MDT meetings.
  7. Implement strategies to reduce hospital readmissions and caregiver stress.
  8. Support patients and families until discharge from G-RACE services.
  9. Refer to community resources as needed.
  10. Maintain infection control practices and document visits and follow-ups.
  11. Participate in MDT meetings and provide education and counseling in primary care settings.
  12. Collaborate with community agencies and conduct staff training on psychogeriatric issues.
  13. Assess elderly at risk of cognitive impairment, dementia, or depression.
  14. Assist in training and research activities related to G-RACE.
Secondary Responsibilities
  1. Assist in data collection and reporting.
  2. Evaluate and improve operational procedures.
  3. Participate in research collaborations.
  4. Perform other duties as assigned.
Job Requirements
  • Bachelor's Degree in Psychology or Nursing with at least 3 years of clinical experience, preferably psychiatric.
  • Good interpersonal and communication skills.
  • Proficient in IT applications.
  • Excellent oral and written communication skills preferred.
