Frontier Healthcare is an established Primary Care Network (PCN) provider, committed to delivering holistic, patient-centred care across our clinics. We invite passionate and dedicated Family Physicians to join our growing medical team in providing high-quality, accessible, and coordinated care to our community under initiatives such as Healthier SG.

Responsibilities:

  • Deliver comprehensive primary care services, including diagnosis, treatment, and management of acute and chronic conditions.
  • Provide continuity of care for patients through long-term management plans, regular follow-ups, and preventive health counselling.
  • Conduct health screenings, immunisations, and chronic disease reviews in accordance with national guidelines.
  • Support the implementation of Healthier SG programmes, including care enrolment, goal-setting, and coordination with community partners.
  • Collaborate closely with the nursing, clinic assistant, and allied health teams to ensure smooth clinic operations and holistic patient care.
  • Participate in multidisciplinary case reviews, quality improvement initiatives, and clinical audits to enhance care standards.
  • Maintain accurate electronic medical records (EMR) in compliance with the Personal Data Protection Act (PDPA) and MOH regulations.
  • Engage in patient education and counselling on lifestyle modification, preventive health, and medication adherence.
  • Uphold a culture of empathy, professionalism, and teamwork within the clinical environment.

Requirements:

  • Medical degree from a recognised and accredited institution.
  • Full registration with the Singapore Medical Council (SMC).
  • Postgraduate qualification in Family Medicine (MMed (FM), GDFM, or equivalent) preferred.
  • Demonstrated experience in chronic disease management (e.g. diabetes, hypertension, lipid disorders)
  • Excellent clinical judgment, communication, and interpersonal skills.
  • Familiarity with MOH programmes such as Healthier SG, CHAS, and Screen for Life preferred.
  • Proficiency in Electronic Medical Record (EMR) systems.
