Care Facilitator

Job Responsibilities
- Fetch clients from their respective homes to the Centre safely, promptly and vice versa
- Ensure clients wear seat belts when seated in the van and all wheelchairs are clamped before the van is allowed to drive off.
- Provide doorstep service as needed to ensure client's safety to and fro their homes/centers
- Build rapport and professional relationships with clients and caregivers
- Report any unusual behaviors of clients to the Reporting Office and the Centre Manager
- Ensure van interior is clean when performing duties. Do infection control duties in accordance to the prevailing MOH advisories such as sanitization of equipment to maintain the required standards of cleanliness and disinfection during service delivery
- Project a professional image as indicated in employee handbook including no wearing of slippers, shorts whilst on duty
- Comply with safety protocols at all times
- Liaise with the Admin Executive and/or designated staff to plan and maintain the transport schedules to ensure optimum route planning to meet clients' transportation needs
- Coordinate with the drivers and to keep track of all the vehicle related statistics and update the records, as required by the Centre and the stakeholders
- Help out in the Centre and support with direct care work, as and when required
- Develop a basic understanding of person-centred care and practice holistic care philosophy
- Administer basic first aid to clients with minor injuries
- Protect clients' confidentiality by adhering strictly to organisational Personal Data Protection Policy and national Personal Protection Data Act (PDPA) and regulations
- Respond timely to emergencies, including crisis situations based on organization's standard operating procedures such as vehicle breakdown or road accident
- Adhere to organizational and service Standard Operating Procedures and service requirements at all times
Requirements
- Secondary School Education
- Preferably some working experience in the healthcare sector
- Knowledge of Mandarin/Malay/Chinese dialects to communicate with the elderly is an advantage
- Able to work well in a team and have passion to serve the elderly
- Good communication and interpersonal skills
