Responsibilities:

  • Build and sustain relationships with GPs and community partners to improve access and quality of care.
  • Support implementation of shared care and community care programs for seamless patient outcomes.
  • Facilitate training, resource sharing, and development of primary care providers' capabilities.
  • Implement frameworks and monitor programs to enhance service effectiveness and meet Healthier SG objectives.
  • Manage partnerships, track outcomes, and represent the organization to strengthen collaboration and visibility.

Job Requirement:

  • Bachelor's degree in Business Administration, or equivalent
  • Minimum 3 years of experience in community care/marketing/business role , ideally in the healthcare sector
  • Candidate with budgeting, operations and administration experience will be advantageous
  • A good team player, meticulous, service-oriented
  • Strong written and verbal communication skills
  • Proficient in Microsoft Office Suite
