Our client is looking for a compassionate and dedicated Social Worker to join the care team.

The role involves providing direct social work services, case management, and emotional support to our clients and their families. You will conduct psychosocial and financial assessments, develop care plans, and work closely with our multidisciplinary team to deliver person-centered care.

Summary:
  • 5 days, office hour
  • West region (Senior Care)
  • $3,500 - $4,500 (based on experience)

Responsibilities:
  • Review referrals and applications from AIC
  • Conduct biopsychosocial assessments for clients and families
  • Build rapport and provide counselling and emotional support
  • Educate families on caregiving and available resources
  • Coordinate care with external agencies (e.g. hospitals, polyclinics)
  • Develop and monitor Individual Care Plans (ICPs) with the care team
  • Maintain accurate case documentation and reports
  • Conduct risk assessments (e.g. suicide risk, elder abuse)
  • Perform financial assessments and assist with subsidy applications
  • Plan and implement client-focused programmes and activities
  • Any other tasks as assigned by management
Requirements:
  • Degree in Social Work/ Post Graduate Diploma in Social Work with 2 years of social work experience
  • Candidates with a Post-Graduate Diploma in Counselling will have an added advantage
  • Effective clinical skills in assessment, intervention, casework and counselling
