Responsibilities:

  • Assist patients with daily living activities, including personal hygiene, feeding, grooming, and dressing, ensuring comfort, dignity, and respect at all times.
  • Implement and carry out individualized care plans developed by the healthcare team to meet patients' physical, emotional, and social needs.
  • Administer medications to patients as per prescribed orders and in compliance with hospital policies, ensuring proper documentation and patient safety.
  • Support patients in activities and rehabilitation programs to promote recovery and overall well-being.
  • Maintain accurate and up-to-date monthly assessment reports on patients' conditions, progress, and any changes in health status.
  • Collaborate with the nursing and medical teams to monitor patient progress, provide feedback, and adjust care plans as needed.
  • Foster a caring and supportive environment for patients, providing emotional and psychological support as necessary.

Requirements:

  • Nitec in Nursing
  • Registered with the Singapore Nursing Board (SNB).
  • Compassionate, empathetic, and dedicated to providing high-quality patient care.
  • Ability to work independently as well as collaboratively in a fast-paced hospital environment.
