Family Support Officer

The Family Support Officer (FSO) fosters positive relationships with families, engaging in regular discussions on children's progress and conducting home visits for targeted support. FSO will design and implement targeted programs for children and families in need, including individualized care plans, while monitoring children's well-being and school attendance. FSO will collaborate with community agencies and maintain accurate records and reports to track child and family progress, ensuring a supportive network for parents and stakeholders.
Key Responsibilities:
Perform Stakeholders Engagement & Partnerships
Establish and maintain positive relationships with parents and family members.
- Engage parents and children on a regular basis
 - Encourage families to participate in parent engagement activities
 - Work with KidSTART Practitioners (KSPs)/ Family Coaches (FCs) if child/family is supported by KSPs or FCs
 - Collaborate with Learning Facilitators/Learning Support Educators (LSEds/Therapists/Educators/parents and other stakeholders to support child and family
 - Tap on available resources to guide parents on child-rearing
 - Do Home Visits for identified children/families
 Conduct Casework
Do intake assessment for all B20 children
- Assess risk and needs of identified children and family
 - Design and implement Individualized Intervention care plan for families at risk
 - Administration and maintenance of case notes / records / confidential files
 - Monitor School Attendance for B20 children
 - Provide support to individual children and families; including advice on external resources for support
 - Do Home Visits for identified children/families
 Deliver Programmes
Develop and manage universal and targeted activities/programmes for children and families from identified group
Perform Community Development Work
Network with community agencies and assist to foster collaborative partnership amongst community, social and corporate partners with PCF
Documentation
Maintain accurate records of each child, family profile and relevant information
- Prepare and maintain reports to track progress and share information with relevant keyholders
 - Administration and maintenance of case notes / records / confidential files
 
Job Holder Requirements:
- Qualification: Recognized Diploma in Social Service/ Social Work/ Diploma in Early Childhood/ Psychology /Related Fields
 - Experience: Minimum 3 years of working experience
 Special Qualities, Knowledge & Skills:
Good knowledge of community resources available to help children & families
- Experience in working with children would be advantageous.
 - Good interpersonal, communications skills (Verbal and written).
 - Due to the nature of work, candidate needs to be proficient in English language, both verbal and written and proficient in a second language (Chinese, Malay or Tamil)
 - Able to multi-task, is resourceful, well organised and meticulous.
 - Possess initiative and ability to work independently and yet be a good team player
 - Have strong interest in developing and caring for children from vulnerable backgrounds and willing to work with their families.
 - Proficient in MS Word, Excel, PowerPoint and Outlook
 - Good team player
