Key Responsibilities

  • Provide psychosocial care to patients and families affected by acute or chronic illness.
  • Deliver individual and family counseling including supportive, grief, and crisis interventions.
  • Conduct interviews and social assessments with patients and families to understand needs and concerns.
  • Identify and help resolve psychosocial, emotional, and practical issues affecting patient care and well-being.
  • Conduct regular social visits to monitor progress and provide ongoing support.
  • Coordinate with healthcare teams to develop and implement patient care plans.
  • Liaise with government agencies and social service organizations for financial assistance, housing, and community resources.
  • Assist in restoring and expanding patients' social support networks.
  • Maintain accurate and up-to-date case records and documentation in accordance with hospital policies.
Requirements
  • Diploma / Degree / Masters in Social Work or a related field
  • Strong interpersonal, communication, and counseling skills
  • Empathy, patience, and the ability to work with people from diverse backgrounds
  • Ability to work independently as well as collaboratively within a multidisciplinary team
  • Familiarity with local community resources and social services
