Responsibilities :
- Deliver social work case management and counseling for clients and their families, including social and financial assessments.
- Review AIC applications and conduct client and caregiver interviews during program or home enrollment.
- Perform biopsychosocial assessments to understand clients' strengths, resilience, and support systems.
- Build strong rapport with clients and families, address concerns, set goals, and encourage positive attitudes. Educate families on clients' physical and psychosocial needs and guide them in accessing resources.
- Manage casework and coordinate with external agencies (e.g., hospitals, polyclinics) to support clients requiring financial assistance for medical care.
- Develop and implement Individual Care Plans (ICP) in collaboration with the multidisciplinary team (MDT).
- Work with PT and OT teams to promote client comfort and confidence.
- Prepare ICP review reports (biannual) and monthly client progress records.
- Assess new and existing clients based on needs, goals, and limitations.
- Maintain accurate documentation, client details, and casework records.
- Provide person-centered counseling for clients and families, including mental health (e.g., depression, anxiety) and adjustment issues.
- Carry out additional tasks as assigned by management.
Requirements:
- Minimum Degree in Social Work, with Registered Social Worker
- Candidate with experience of social work for elderly will be a plus
