What We're Looking For

Trillium Health Resources has a career opening for an Outreach Diversion Specialist to join our team This role is a vital force in facilitating access to critical services. You will manage the Referral Screening Verification Process (RSVP), ensuring individuals are seamlessly connected to the support they need. The Outreach Diversion Specialist plays a crucial part in proactively assessing and responding to the health and safety needs of TCL members ensuring continuous, high-quality care and support.

On a typical day, you might:

  • Screen members that are being considered for Independent Housing through the Transition to Community Living Initiative (TCL) or Admission to an ACH.
  • View each RSVP referral within 72 hours of the submission date.
  • Ensure that all TCL eligible members are fully informed of all available alternatives to entry into an ACH.
  • Provide education about all available services, natural and community support, treatment options, diagnosis, etc.

Qualifications

Required:

  • High School Diploma or GED. 
  • Be a person with one (1) year lived experience with a behavioral health condition and/or substance use. *Lived Experience is defined as living in recovery with mental illness and/or substance use disorder.
  • Required to be a North Carolina Certified Peer Support Specialist, which may be attained within six (6) months of commencing employment with Trillium. The NCCPSS Program requires all applicants for certification to meet the following Program eligibility requirements:
    1. Must be 18 years or older
    2. Have lived experiences and in recovery from a serious mental illness and/or substance use disorder
    3. Must be in recovery for at least ONE year 
    4. Have a high school diploma or equivalent
  • Must have a valid driver's license.
  • Must reside within Trillium's catchment area of North Carolina, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.

Preferred:

  • North Carolina Certified Peer Support Specialist.
