Our esteemed client in the healthcare industry is hiring an experienced Medical Social Worker.

  • 2 days WFH per week

Responsibilities:

  • Manage casework involving comprehensive assessment and intervention to support the well-being of patients and their families.
  • Coordinate referrals to appropriate service providers and conduct home visits when required.
  • Collaborate closely with members of a multi-disciplinary healthcare team.
  • Provide support for ongoing projects and community programmes.
  • Participate in relevant training sessions and casework supervision.
  • Perform any other duties as assigned by reporting officers.

Requirements:

  • Degree in Social Work or a Graduate Diploma in Social Work
  • Registered with Singapore Association of Social Work
  • Experience in means testing and healthcare setting is advantageous
  • Good interpersonal skills
  • Comfortable to conduct home visits.
