Senior Staff Nurse, Chronic Diseases Management

As a Senior Staff Nurse (Chronic Disease Management), you will be responsible for the following duties:

  • Identify and enroll patients with chronic conditions (e.g., diabetes, hypertension) who need care management.

  • Perform regular assessments to understand patient needs and disease progression.

  • Track clinical indicators such as blood sugar, HbA1c, and blood pressure to manage chronic conditions effectively.

  • Develop, implement, and adjust individualized care plans based on patient progress and clinical targets.

  • Conduct and interpret relevant chronic disease tests and ensure equipment is accurate and well-maintained.

  • Facilitate referrals and transitions between primary care, specialists, and hospitals to ensure continuity of care.

  • Educate and support patients through self-management programs, workshops, and support groups.

  • Maintain accurate, confidential records of all care management actions and patient progress.

  • Supervise, mentor, and train junior care managers and nurses, including performance appraisals.

  • Participate in audits and service evaluations to improve chronic disease management.

  • Analyze program outcomes and use data to improve patient care and system efficiency.

  • Apply research findings and support evidence-based protocols in chronic disease care.

  • Monitor and follow up with patients who miss appointments or deviate from care plans.

  • Propose and implement improvements in care coordination, workflows, and referral systems.

  • Represent chronic disease care in internal and external forums and support service development.

Requirements:

  • Registered with the Singapore Nursing Board.

  • Possess 5 to 7 years of clinical experience, including 2 to 3 years specifically managing patients with chronic conditions
