URGENT Staff Nurses

Salary Range: SGD 3,000 – 4,500++ per month
Working Hours: Monday to Friday, 8:30 AM to 5:30 PM
Allowances:
- On-call allowance
- Fixed monthly community care nursing allowance
Position 2: Staff Nurse – Chronic Illness Home Care
Key Responsibilities (Main):
- Conduct home visits for assigned patients.
- Perform comprehensive nursing assessments and initiate individualized care plans.
- Educate caregivers and families to support continuity of care at home.
- Coordinate with doctors for follow-ups and care decisions.
- Maintain accurate documentation and safeguard patient confidentiality under PDPA guidelines.
- Identify and refer patients/families for financial or psychosocial support as needed.
- Participate in organizational fundraising activities as required.
Additional Responsibilities (Palliative Care):
- Adjust and titrate medications under physician supervision for symptom relief.
- Provide end-of-life care guidance and bereavement support.
- Refer patients to inpatient hospice or specialized palliative services as necessary.
- Target: Minimum 6 patient visits per day.
Additional Responsibilities (Chronic Illness Home Care):
- Monitor medication compliance and vital health signs.
- Perform nursing procedures such as wound dressing, catheter insertion, and NGT care.
- Target: Minimum 7 patient visits per day.
- Liaise closely with referring institutions.
Requirements:
- 1–3 years of relevant nursing experience.
- Diploma or Degree in Nursing (or equivalent).
- Certification in Palliative Care Nursing.
- Experience in palliative medication titration (e.g., morphine, fentanyl) is highly desired
